In addition to PLAYER INFORMATION section (left), Player must also complete and submit ALL applicable Permission Forms (download links below) to rapcewicz.jason@gmail.com
Player will not be allowed to enter ice surface until all associated paperwork is on file.

Main Camp information package will be forwarded by email.
Registration for "Out of Town" players will be Friday August 23rd at 5:00 PM.
Registration for "Local" players will be Saturday August 24th at 9:00 AM.
